HOME Study

The Home based Outreach chronic disease Management Exploratory Study (HOMES Study) explores novel approaches to address chronic disease management in home-based outreach settings.

Aim

young aboriginal coupleThis study aims to develop and implement a patient-centred, multidisciplinary, outreach model of care in primary health care to improve the health outcomes and quality of life of Aboriginal and Torres Strait Islander people with chronic disease.

Research Questions

The study has five key research questions:

  1. Is this model of care feasible, acceptable and appropriate to Aboriginal and Torres Strait Islander people with chronic disease and their primary health care service?
  2. Does this model of care improve health outcomes and quality of life of Aboriginal and Torres Strait Islander people with chronic disease?
  3. How does a primary health care service adapt and implement this model of care?
  4. What are the key elements of this model, and how does it differ from usual chronic disease care provided by the primary health care service?
  5. How do the participants’ social contexts affect their health and wellbeing and their ability to engage and be supported in their chronic disease management?
